The Wholistic Health Map

Every new patient at Wholistic MD completes The Wholistic Health Map — our comprehensive online health assessment — before the first consultation. Dr. Dusa reviews your complete health story in advance, so your visit begins with the root causes rather than the paperwork.

The Health Map was built in-house by our practice on the Institute for Functional Medicine’s clinical framework. It covers your health history, current symptoms, medications and supplements, lifestyle, and family patterns.

What to expect

Plan on about 45–60 minutes in total. You can save your progress and return as often as you like during your access window, so it doesn’t have to be done in one sitting.

Our office will email you a personal invitation with your activation link. Use it to create your account, then log in to begin.
